Preparing for your visit

To prepare for a successful office visit with us, there are a few things to know:

What to bring to your visit

For your first visit to our office, please come to your appointment 15 minutes early to fill out key paperwork, such as insurance information. Please bring along the following items:

  • Photo identification (such as a driver's license)
  • Insurance card and information
  • A list of your current medications, medications you no longer use and any treatments or procedures you've had in the past year
  • Medical records from your primary care provider or other health provider
  • Images or scans

Patient forms

Save time by completing, downloading and printing your patient forms and bringing them with you to your appointment.

Billing and insurance

Once we have your accurate and complete insurance information, we’ll submit claims for payment to your insurance carrier to pay for your visits, treatments or services. Your insurance may hold you responsible for any co-payments, co-insurance or other out-of-pocket costs. Learn more about insurance plans we accept.

We no longer accept cash for payments made in our offices. You can pay with your credit or debit card or personal check.

digiChart Patient Portal

Our patient portal can help make managing your care easier. Get updates from your provider after your visit, fill out health forms, and view lab results all from the comfort of your home.

Before your first appointment with Women's Health Alliance, you'll receive a mailed letter with instructions on how to access the digiChart portal online.

Sign up for digiChart access

  • Go to the digiChart Patient Portal site.
  • Sign in with the username and password you received in the letter mailed to you before your visit.
  • Please fill out as much of your patient history information as you can. If you don't remember a specific date or don't know certain pieces of your medical history, please move on to the next step of questions.

Office policies

Our aim is make your experience as a patient with us as easy as possible. The following are our office rules and policies:

Appointments

To make an appointment with Women's Health Alliance, call our offices at 214-824-3200 or 972-613-6336. Our offices are open Monday through Friday, from 8:30 a.m. to 5 p.m. You can also make appointments through the Patient Portal.

After-Hours Care

If you are experiencing a life-threatening medical emergency, please dial 911 or go to your nearest emergency room. If you need care before or after business hours, your call will be forwarded to our answering service and on-call doctor to help you.

Prescription refills

If you need a medication prescription refilled, please call our office during business hours. We may ask you to come in for a visit with a provider before you can request a refill. It may take at least 24 hours to complete your refill request.

Billing and payments

Your health insurance provider can pay for your visit, but we also accept cash, personal check and most credit and debit cards. You can also make a payment online.
